• How do I apply for support from HAAPI?

    How much time will it take to process my application?

    We estimate up to four weeks once the completed application is received along with all the necessary documents.

    What do HAAPI funds cover?

    HAAPI funds may cover one or two traditional or bone-anchored hearing aids depending on what is most appropriate for the hearing loss. HAAPI also covers a one-time fitting fee and pediatric care kit, as well as one earmold per hearing aid or one unilateral or bilateral softband.

    How much will I have to pay for my child’s hearing aids?

    Families will be responsible for a portion of the total cost of hearing aids based on family size and income. Every family will pay between $100 and $500 per hearing aid, depending on level of cost participation. This level is calculated based upon household size and income.The cost participation scale can be found here. Parents will be able to use the scale to determine their level of cost participation before submitting an application.

    After being approved for hearing aid(s), an invoice will be sent to the family. Payment must be received within thirty (30) days or the application will be closed. Payment can be made online via credit card or by check through the mail.

    Which devices are covered?

    Participating audiologists can select models from several prominent manufacturers. These include traditional hearings aids as bone anchored hearing aids.

    Are there out-of-pocket costs for dispensing fees if HAAPI funds are used to purchase the hearing aid?

    No. There are no out-of-pocket costs for dispensing fees for the families receiving assistance through HAAPI. However, your audiologist may charge you to complete a hearing evaluation if needed. After the initial fitting, your audiologist may bill/charge you their usual and customary rates for follow-up care and and hearing aid adjustments.

    What do I do if my child currently does not have an audiologist?

    If your child is not currently established with an audiologist, you may choose a participating HAAPI provider from the Registered Audiologists. To receive assistance from HAAPI, you must work with a Participating Audiologist.

    Can I stay with my child’s current audiologist?

    To receive assistance from HAAPI, you must work with a participating HAAPI audiologist. A complete list of providers is available at HAAPindiana.org. If your child’s audiologist is not currently participating in HAAPI, you may either choose another participating audiologist from the list, or you can ask your current audiologist to become a registered audiologist at no cost. The application to become a participating audiologist can be found on our website.

    What do I need for age verification or proof of school enrollment? What if my child is homeschooled?

    A birth certificate for children who have not yet in kindergarten or IEP, report card from current school year, or letter from school stating enrollment for school age children. Children who are homeschooled are also eligible to receive assistance from HAAPI. If you child is homeschooled, a copy of homeschool enrollment with the state of Indiana will be required.

    Will I have to verify income?

    Yes. You will need to provide three consecutive pay stubs for all working adults in the household. If self-employed, you must provide your most recent tax return.

    How do I get medical clearance for hearing aids?

    Medical clearance for hearing aids must be provided by a Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.). Often, a child’s pediatrician, family doctor, or ENT will be comfortable signing the medical clearance. If you need assistance obtaining medical clearance, ask your Participating HAAPI Audiologist.If your physician doesn’t have his/her own form, click here to download our medical clearance document. Medical Clearance forms must be signed within six months of the application date.

    Is there a minimum hearing loss required to receive HAAPI funds?

    There is no minimum loss required to receive HAAPI funds. An audiologist must recommend hearing aids.

    Do HAAPI funds cover follow up services for hearing aid maintenance?

    HAAPI does not cover follow-up services for hearing aid maintenance. However, the warranty covers any repair (should a hearing aid need to be sent back to the manufacturer for service) for the first 3 years. You should check with your Participating HAAPI Audiologist to find out about ongoing hearing aid maintenance fees.

    What should I do if my child loses their hearing aids?

    All hearing aids awarded through HAAPI will have a 3-year warranty that covers one loss time during the warranty period. You will owe the deductible or a nominal fee. Contact your audiologist if you need to use this coverage.

    How often can I apply for HAAPI funds to cover new hearing aids for my child?

    Generally, hearing aids should be replaced every three to five years. Recipients of HAAPI are eligible to apply for assistance every three years.

    What is prioritization given to applicants of HAAPI funds?

    Indiana code 16-35-8 specifies prioritization of HAAPI funds for children ages 3-13 who have not yet graduated high school. HAAPI Administrators will continuously monitor applicants ages and reserve the right to pause approvals temporarily for children 14 and over at any time to ensure prioritization occurs.
